The UPMC Palliative and Supportive Institute collaborates with Family
Hospice to help patients meet the end of life with dignity and grace in a
compassionate environment. As Pennsylvania’s largest hospice provider,
Family Hospice serves many counties in central and western Pennsylvania.
Services are available 24 hours a day, seven days a week in a variety
of settings, including private homes, personal care and nursing homes,
hospitals, and our two inpatient care centers located
in the city of Pittsburgh on the Canterbury Place campus in Lawrenceville
and the other in Williamsport, Pa.
Most of the care we provide takes place in the patient’s home.

based on the specific needs of each client.
Greenbriar also has several different locations across Pennsylvania, that
offer similar addiction treatment programs and services.
The residential treatment program is able to offer support and a healing
environment, where each individual is able to work with their treatment
team and peers. Most of the treatment options offered include individual
therapy, group therapy, medication management, family education, and
relapse prevention training. All clients will also participate in
aftercare planning where they are able to receive long-term support.

Adolescents and Adults
Comprehensive, whole-person drug and alcohol addiction treatment
services to individuals in the Pittsburgh area. The Green Tree location
provides outpatient treatments to men and women struggling with
substance abuse, but more specifically opioid addiction. Clients will
have access to medication assistance, various behavioral therapies, and
EMDR. Though these programs are more flexible, clients will be required
to participate in a certain number of hours of treatment each week. As a
more evidence-based facility, treatment plans might also include
cognitive behavioral therapy, DBT, and psychodynamic therapies. If
clients require other levels of care or specific programs offered at a
different Pennsylvania Gateway Rehab location, they will be referred to
one of those facilities in order to receive the treatment they need.

Tadiso is a medication-assisted treatment facility that provides
several types of care to adult men and women over the age of 18, who
struggle with opioid addiction and the withdrawal symptoms or cravings
that often accompany it. The specific medications they offer that address
these symptoms include methadone, buprenorphine, and Naltrexone, all of
which are FDA-approved and can help motivate clients to commit to their
outpatient program. Clients will also be expected to participate in both
individual and group therapy, and they may also have access to treatments
like reality therapy, CBT, Gestalt therapy, and experiential activities.
The goal of these treatments is to address underlying conditions, help
clients develop coping skills, hold clients accountable for their
abstinence, and participate in relapse prevention training. Other
available treatment options include art therapy, recreational activities,
relaxation training, and therapeutic fitness.

Along with being a medication-assisted recovery center, Jade
Wellness does also provide several different outpatient programs that
provide more comprehensive support to men and women dealing with any type

They provide services to adults and adolescents in order to keep programs
accessible throughout the community for all who need help.
Individual therapy, group therapy, family programs, and aftercare
planning. Clients struggling with opioid withdrawal will be prescribed
either Vivitrol, Sublocade, or Suboxone.

The Persad Center provides different services to members of the LGBTQ+

Their main goal is to provide education, prevention services, community
outreach, training, and counseling to individuals in this community who
were previously denied equal access to care. The intensive outpatient
program is based on the Matrix Model and combines several different
modalities, including group therapy, individual therapy, medication
management, CBT, contingency management, 12-step facilitation, and skill
development classes. Clients will also have access to other counseling
services, HIV education, and crisis interventions. The Persad Center does
accept various health insurance plans, forms of self-pay, Medicare, and
Medicaid.

needs. Their peer support group is one of the most critical pieces of each
treatment plan. It allows clients to connect with one another, form
trusting relationships, and learn various skills from their peers that can
help treat those underlying or co-occurring disorders that may have led to
drug or alcohol use. This also helps hold each person accountable for
their recovery process.

Sojourner House provides faith-based
treatment services to women who have children. Not only do they help treat
substance abuse, but treatment teams also work with each woman to repair
relationships with their children while they are recovering. Their
residential drug addiction treatment program allows women to live with
their children while also receiving intensive treatment.
Clients must be at least 18, and if they have children, the children must
be under the age of 12 in order to remain in the facility with their
mother.
The length of treatment is variable, but clients can remain in this
facility for up to six months.
Each program is totally individualized to the client and may include
various forms of individual or group therapy, women-specific care, and
family programs.
Pittsburgh Comprehensive Treatment Center (Formerly Discovery House)

sobriety, communication, coping skills, and more.

residential facility that offers long-term care to each individual. The
goal of this program is to treat whatever drug use the client suffers from
while also addressing any co-occurring medical conditions or underlying
mental illnesses that they may also be dealing with. Clients will also
receive access to housing assistance programs or employment assistance in
order to help re-establish their lifestyle in the most successful way
possible.

Assisted living communities typically offer both
semi-private and private rooms, a strong activity schedule, social
opportunities for residents, and will have communal areas. Assisted living
is a form of senior care that provides residents with supportive services
and assistance with the activities of daily life, and includes things like
bathing, eating, hygiene, and dressing. Assisted living services may also
include basic first-aid and assistance with medication management. It does
not include regular medical care, rehabilitation services, or skilled
nursing care.
The primary focus of nursing homes is resident health, while assisted
living communities are intended to support relatively active and
independent seniors.

Program - Prescription Assistance
Pennsylvania's prescription assistance programs for older adults, PACE and
PACENET, offer low-cost
prescription medication to qualified residents, age 65 and older. The
program works with Medicare Part D plans and other prescription drug plans
such as retiree/union coverage, employer plans, Medicare Advantage (HMO,
PPO) and Veterans’ Benefits (VA) to lower out-of-pocket costs for
medications.
PACE and PACENET eligibility is determined by your previous calendar
year’s income.

- You must be 65 years of age or older.
- A Pennsylvania resident for at least 90 days prior to
the date of application.
